What to expect when you book your physician appointment
We accept most major insurance plans, including Cigna, Humana, Medicare, Tricare, UnitedHealthcare, Aetna, and Blue Cross Blue Shield. We also offer in-office payment options. Booking is simple. You can call our West Melbourne office on (321) 491-2842, use our online booking system, or send an enquiry through our website. We encourage booking in advance, though walk-ins are welcome depending on availability.
To make your first visit smooth, please bring:

Dr. Christopher Thaver, MD is a board-certified physician recognised by the American Board of Family Medicine. He graduated with honours from the University of South Florida and completed his medical training at Morehouse School of Medicine, followed by a Family Medicine residency in Georgia. Born and raised in Melbourne, Florida, he returned home to care for the community that raised him, with clinical interests in preventive health, weight loss, hormone therapy, and longevity medicine.
Our clinic is open Monday to Friday, 8am–5pm.

FMLA & Referrals
Happy to help with FMLA
FMLA paperwork may be completed at your provider's discretion if your medical condition qualifies for leave under FMLA guidelines. A processing fee is required before any paperwork is completed. This fee covers the time needed to review your medical records, complete the required forms, and address any additional paperwork or follow-up requests related to your claim.
How long do referrals take? Most referrals are submitted within 24–48 business hours after your visit. If your insurance plan requires prior authorization, such as many HMO plans, approval may take up to one week.Once your referral has been sent, the receiving specialist's office is responsible for reviewing it and contacting you to schedule an appointment. This process may take 1–2 weeks. If you have not been contacted after two weeks, please reach out to our office and we will be happy to assist.
Work and school notes are provided at your provider's discretion when medically appropriate and based on your evaluation.Do you complete other medical forms? Other forms may be completed at your provider's discretion. A $20 processing fee is required before forms are completed to cover the time needed for provider review and documentation.
Billings, Fees and Payment
How The AVIA Total Health Team help you
As a participating provider with your insurance plan, we are required to collect the patient responsibility determined by your insurance company for services provided. We do not charge more or less than the amount your insurance has assigned as your responsibility.Please note that balances remaining unpaid for more than 180 days may be referred to collections, and a $150 collections fee may be added to the outstanding balance.
Missed appointments and late cancellations are subject to a fee. Your appointment time is reserved especially for you, and when an appointment is missed without enough notice, we often are not able to offer that time to another patient who may need care. Letting us know in advance if your plans change helps us continue providing timely care for everyone.

Medications, Refills & Controlled Substance Policies
How do I request a medication refill?Please request refills at least 3 business days before you run out of medication. Refill requests can be submitted through the Patient Portal or by emailing medrefill@aviatotalhealth.com.Refills are processed during normal business hours only. Requests received after hours, on weekends, or holidays will be addressed on the next business day. To avoid interruptions in your medication, please plan ahead.
How are controlled medications managed?Controlled medications, including medications such as ADHD stimulants, are prescribed in accordance with state and federal regulations. Patients taking controlled medications are required to be seen monthly for continued treatment.Periodic urine drug screening may also be required as part of routine monitoring. If your insurance does not cover the cost of the screening, you may be responsible for the associated fee.Early refills, replacement prescriptions for lost or stolen medications, and after-hours controlled substance refills cannot be provided.
Do you prescribe ADHD medications?We follow all current state and federal guidelines for prescribing ADHD medications. If you are transferring care from another provider, your medical records may be required before treatment can be continued. Some patients may be referred to psychiatry based on their medical history or current treatment plan.
Will you complete a prior authorization for my medication?Yes, when medically appropriate, we will submit prior authorizations on your behalf. While we are happy to assist with this process, insurance approval is determined solely by your insurance company. We recommend checking your prescription benefits to confirm medication coverage and available alternatives if needed.
